Welcome! Please complete this form and upload your proofs of residency documents if you are:

  1. Moving within your current Forsyth County school's zone (Locate Schools by Address)

  2. Moving within Forsyth County during the summer (Locate Schools by Address) - Please submit your change of address the last day of school or during the summer.

  3. Moving and changing schools within Forsyth County during the school year (Locate Schools by Address)

    1. Please contact your child's current school to make an appointment to complete a withdrawal form. List of Forsyth County Schools
    2. Please contact your child's new school to make an appointment to provide your completed withdrawal form to enroll. List of Forsyth County Schools

Please complete the information below. Copies of your valid proofs of residency and judge signed custody documents if applicable are required. The name and service/physical address of the property must be visible on both proofs of residency submitted. Please contact your child's school or the Student Information Systems Department at 678-947-0863, ext. 5 if you have any questions or need further assistance with completing your Change of Address.

   Forsyth County Schools Board of Education Policy JBCAA: The property address given must be the actual location where the student and parent/guardian live full time. A parent/guardian who owns property in the county, but does not reside in the county, is not considered a resident for purposes of this policy. A family that rents a home in Forsyth County, while owning a home in another county or in a different school attendance zone in Forsyth County, solely for purpose of providing residence for their children to attend school in Forsyth County or in a different school attendance zone will be considered in violation of this policy. Students who are illegally enrolled shall be immediately withdrawn. Violations of this policy may be pursued in court.
